How MTM works
Step 1
Participants bring all prescription and over-the-counter medications and supplements to an onsite appointment.
Step 2
Our pharmacist evaluates appropriateness, side effects, interactions, duplications, adherence, and formulary use.
Step 3
Together they identify, prioritize, and solve actual or potential issues, with education and goal setting.
Step 4
Dietitian visits about every three months monitor weight, BMI, blood pressure, cholesterol, and A1c as indicated.
Let's talk about a program built around your people and your budget.
